Day Habilitation Programs

PDI operates day facilities in both Dover and Clarksville. Individuals outcome measures are the criteria used to develop their individualized program for services. The Adult Day Habilitation Programs provide services to adults age 22 and older with a diagnosis of intellectual disabilities. Services include assessments, developmental and vocational training, behavior management, transportation, follow-along, community service employment contracts and federal contracts at Ft. Campbell.

Supported Living Programs

PDI also provides residential support services throughout Montgomery and Stewart Counties. While our primary staffing model is a 24 Hour Shift Staff pattern, we have homes that are staffed with Live-In Companions as well as Family Based model homes. The staffing pattern is determined by the individual's specific needs and the considerations of the Circle of Support (those closest to the individual served make up the Circle of Support).

Services include, but are not limited to:

  • Daily living skills training
  • Socialization skills
  • Self-help skills
  • Community integration skills and residential care services provided from 24-hour care to limited care as appropriate and required

Who do we Serve?

  • Adult ServicesAdults age 22 or older
  • Adults with a diagnosis of mental retardation as defined by the American Association of Intellectual and Developmental Disabilities with an IQ less than 69
  • Approval by Department of Mental Health and Mental Retardation

Buddy Ball provides individuals with a mental or physical disability the opportunity to sports, regardless of their limitations. Buddy Ball helps individuals learn teamwork and promotes self-esteem, one of the tools needed to help overcome the stigma often associated with a disability.

Kids Depot

Established in 1999, Kids Depot is a state licensed facility providing before and after school care for elementary school age children ages 6 weeks to 12 years. We have a mini school bus for transportation, and currently serve Sango Elementary & East Montgomery Elementary.
